Question 2: who is the group for?
If your group serves a specific type of person, use a short qualifying question. Keep it relevant to the community rather than turning the application into a survey.
Question 3: offer the next step
If email follow up is part of your strategy, use one slot to offer something specific and ask for an address. Explain what you will send. Do not hide the purpose of the question.
Short questions usually win
